Folate (Vitamin B9)

Educational profile — independent, evidence-informed overview. This content is not medical advice. Please consult your healthcare provider for personalised guidance.

🧬 Family & Essentiality

FamilyWater-soluble B-vitamin
EssentialityEssential — must be obtained from diet
Names/FormsFolate (food forms), Folic acid (synthetic), 5-MTHF (active form)

🌿 Plain-Language Summary

Folate carries one-carbon units for DNA synthesis and methylation. Adequate folate status is crucial during periods of rapid growth and before/early in pregnancy to support normal neural tube development.

⚗️ Molecular & Chemical IDs

  • Folic acid: C₁₉H₁₉N₇O₆ (CAS 59-30-3)
  • 5-MTHF: Various salts (e.g., calcium L-methylfolate)
  • Units — DFE (Dietary Folate Equivalents):
    • 1 µg DFE = 1 µg food folate
    • 1 µg DFE = 0.6 µg folic acid (from fortified foods/supplements with meals)
    • 1 µg DFE = 0.5 µg folic acid (empty stomach)

⚡ Functions

  • DNA/RNA synthesis: Purine & pyrimidine formation for cell division and growth
  • Remethylation: Homocysteine → methionine (requires vitamin B12)
  • Methylation support: Provides 5-MTHF for methylation reactions across tissues

🍽️ Absorption & Bioavailability

Folic acid (synthetic)High bioavailability; used in AU wheat flour fortification for bread-making
Food folatePolyglutamate forms require deconjugation; lower bioavailability; cooking losses occur
5-MTHFBiologically active; useful where folate metabolism or conversion is impaired

Practical tip: Minimal water + gentle cooking (steam/microwave) helps retain folate in vegetables.

🇦🇺 Australian NRVs (NHMRC)

  • Adults: 400 µg DFE/day
  • Pregnancy: 600 µg DFE/day
  • Lactation: 500 µg DFE/day

UL (adults): 1,000 µg/day for folic acid from fortified foods/supplements (does not apply to food folate).

🤰 Periconception Guidance (General)

Many health authorities advise a daily folic acid supplement for people who could become pregnant, beginning at least 1 month before conception and continuing through the first trimester, to reduce the risk of neural tube defects. Individual needs vary — seek personalised medical advice.

🚨 Deficiency & Excess

Deficiency

  • Megaloblastic (macrocytic) anaemia, glossitis, fatigue
  • Homocysteine may be elevated
  • Risk ↑ with alcohol misuse, malabsorption, low vegetable intake, certain medicines

Clinical caution

High folic acid intake can mask vitamin B12 deficiency. Investigate B12 if macrocytosis or neurological symptoms are present.

🥗 Food Sources

  • Leafy greens (spinach), asparagus, avocado, citrus
  • Legumes (lentils, chickpeas), beans
  • Fortified breads & breakfast cereals (AU: mandatory folic acid fortification of wheat flour for bread-making)

🧪 Testing & Monitoring

  • Serum folate: reflects recent intake
  • Red cell folate: reflects longer-term status
  • Homocysteine: functional marker influenced by folate/B12/B6

🔗 Interactions

  • Methotrexate: antagonises folate pathways; folinic acid (leucovorin) may be used as rescue under medical supervision
  • Some anti-epileptics (e.g., valproate, carbamazepine) and metformin can affect folate status
  • Alcohol misuse and chronic gastrointestinal disorders can reduce status

Genetics: Variants in folate enzymes (e.g., MTHFR) can influence folate metabolism; clinical interpretation should be guided by a healthcare professional.

📊 Evidence Snapshot

  • 🔬✅ Well-established: Adequate periconception folate reduces neural tube defect risk
  • 🧪⚖️ Clinical: Treat folate deficiency after confirming B12 status
  • ⚠️❌ Unproven/hype: High doses in the general population without indication
